Certificate Forms The online system allows requesters to request certificates of insurance for their departments and locations. There are six types of certificates: • Professional Liability - Individual • Professional Liability – Institution • Professional Liability – (Non-Medical) • General/Automobile • Property • Workers’ Compensation After requesting a certificate of insurance the user will be notified when the request has been approved or rejected. If a request is rejected, accompanying notes explain why.
